OUR RESPONSIBILITIES

K-Counseling & Anxiety Treatment LLC is required by law to maintain the privacy and security of your Protected Health Information (PHI), provide you with this notice, follow its terms, and notify you if a breach occurs.

USES AND DISCLOSURES

Your PHI may be used for treatment, payment, and healthcare operations, including scheduling, documentation, billing, quality assurance, supervision, and coordination of care.

ELECTRONIC HEALTH RECORDS

We use Jane Practice Management Software, a HIPAA-compliant system, for scheduling, documentation, secure communication, and billing. Access is limited to authorized personnel only.

TELEHEALTH & ELECTRONIC COMMUNICATION

Telehealth services and electronic communication may be used with your consent. AI-assisted documentation tools may be used only with documented consent.

YOUR RIGHTS

You have the right to access your records, request corrections, request confidential communications, request restrictions, receive an accounting of disclosures, and receive a copy of this notice.

COMPLAINTS

You may file a complaint with K-Counseling or the U.S. Department of Health and Human Services without retaliation.
